Child Care Safety Portal

The web portal is a resource for Oregon residents to get information about the safety, health, and quality of licensed childcare facilities. Searchable content includes facility status, a summary of the state’s inspections of the facility, complaint information, enforcement activity, and details about injuries and deaths that have occurred at the facility. Emergency suspensions

The state can suspend a license when it finds an immediate danger to the health or safety of children. Click here for a list of licenses that are currently suspended.

Injury Log

This injury log includes injuries that occurred at licensed child care facilities where attention from a licensed health professional, such as a physician, EMT, or nurse was sought. Facilities are required to report these injuries to the Office of Child Care within seven days of the occurrence.  Click here for details about each serious injury that occurred at a licensed child care facility in the past year.

Child Care Safety Portal

The web portal is a resource for Oregon residents to get information about the safety, health, and quality of licensed childcare facilities. Searchable content includes facility status, a summary of the state’s inspections of the facility, complaint information, enforcement activity, and details about injuries and deaths that have occurred at the facility.

Every regulated facility receives at least one announced inspection and one unannounced inspection every year. The portal displays inspections completed in the past 3 years and facility status for the past 10 years. Only valid complaint allegations and observed non-compliance are displayed.

Valid A complaint is found valid when there is evidence that the violation occurred.  The information displayed online only includes complaints that were found valid and violations that were observed as part of the on-site complaint assessment.
Valid & Corrected The rule(s) that were violated will be listed online, and it will be noted whether the violation was corrected during the on-site complaint assessment.

The information in the Child Care Safety Portal is updated every Tuesday morning by 8am PST.

Request for Compliance History

An individual inquiring about the compliance history of a home-based child care can call the Office of Child Care’s Central Office at the 503-947-1400 or 1-800-556-6616.

An individual inquiring about the compliance history of a center-based child care can call the facility’s Licensing Specialist directly or the Office of Child Care’s Central Office to obtain the name and phone number for the facility’s Licensing Specialist. The name of the Licensing Specialist for the facility would be displayed on the facility information screen.

Emergency Suspensions

The Office of Child Care has suspended the following licenses. For additional information, including a list of compliance history with injury reports, legal actions and more, please contact the Office of Child Care at 1-800-556-6616.

If a license was previously listed on the Emergency Suspension list and no longer appears it may indicate:

  1. The Emergency Suspension action was lifted and is no longer in place; or
  2. The license has expired and the issue that led to the Emergency Suspension has not been resolved by the Office of Child Care.

Emergency Order of Suspension are redacted in accordance with ORS 181.534, 192.435, 192.437, 409.255, 419B.035(7) and OAR 414-061-0040(2) and will be posted when available.
